Attention Students Who Will Do Professional Practice Internships in Public Hospitals affiliated to Istanbul Provincial Health Directorate in the Spring Term of 2021-2022!
1. Students who are approved by the Istanbul Provincial Health Directorate to do a Vocational Practice internship in the Spring Term are required to learn the documents required for internship registration (the students whose names are mentioned in the Announcements section of the University's SHMYO Web Site) from the institutions and apply with these documents to the relevant institution where they will do their internship. .
2. The insurance entries of the students who will take the Vocational Practice that we have placed in the state hospitals affiliated to the Istanbul Provincial Health Directorate will be made by the school. They can request it from the Accounting Unit (Block A) by sending an e-mail to the address or coming to the school.
3. Accordingly, in order for our students to complete their procedures (if you still have not signed the Professional Practice Internship Forms), the "Vocational Practice Internship Form" prepared in 3 copies on the computer between 07.03.2022 and 11.03.2022, between 07.03.2022 and 11.03.2022, and "Internship Contract" if the institution wants ” and get a wet signature from the Program Director (from the SHMYO Internship Commission on days when your Program Director is not at school) (Block C). After getting the signature of your Program Director (C Block), they can get approval from the SHMYO Directorate (C Block 2nd Floor Secretariat) and go to the institution to have their insurance entry documents and internship forms signed from the Accounting Department. After getting the signature from the institution; One copy of the internship forms must be submitted to the student affairs office, one copy to the institution where the internship will take place, and one copy to the head of the program.
CAUTION! The names of all our students were sent to the Provincial Health Directorate and recorded in the internship software. If your name has not been forwarded to the institution yet; You can ask the person(s) responsible for the training unit of the hospital to check the internship software or to contact the regional directors of the Provincial Health Directorate.
*Documents of students who will do internship at state hospitals affiliated to Istanbul Provincial Health Directorate, which are sent to shmyostaj@gelisim.edu.tr via e-mail, will NOT be accepted.

FORMS AND DOCUMENTS:
1. Professional Practice Internship Form (3 copies) (To be submitted to the Internship Institution, Program President and Student Affairs.)
2. Internship Contract (It should be prepared depending on the hospital's request. State Hospitals want it.)
