Attention Students who will do their Summer Internship/Vocational Training in Public Hospitals affiliated to Istanbul Provincial Health Directorate in the Summer Term of 2021-2022!
Students who are approved by the Istanbul Provincial Health Directorate to do internship in the Summer Term (the list below contains the information of these students) learn the documents required for internship registration (OHS, Committee Report, etc.) from the relevant hospital or the website of the relevant hospital, and together with these documents, the requested hospital is requested. must complete their application by submitting all documents before the internship start date.
The insurance entries of the students who are on the list who will take vocational training in the summer term, which we have placed in state hospitals affiliated to the Istanbul Provincial Health Directorate, will be made by the school, and they can check their insurance entry documents via e-state as of Tuesday, June 21, 2022. Students whose insurance entry document does not appear in the e-government system can come to the school and request an insurance entry document from the Accounting Unit (A Block).
Accordingly, in order for our students to complete their procedures, they come to the school together with the "Summer Internship Form", which they prepared in 3 copies in computer environment between Tuesday, June 21, 2022 - Friday, June 24, 2022, between 10:00 and 16:00, and the "Internship Contract" if the hospital wants them, and ask the Program Director (your Director of the Programme). On days when they are not at school, they must obtain a wet signature from the SHMYO Internship Commission (Block C). After getting the signature from the Head of the Program (Block C), approval from the SHMYO Directorate (You can apply to the C Block 2nd Floor Secretariat) and going to the hospital with the insurance entry document obtained from the Accounting Department must be signed and stamped on the internship forms. After getting a signature-stamp from the hospital; One copy of the internship forms must be submitted to the student affairs office, one copy to the institution where the internship will take place, and one copy to the head of the program.
CAUTION! The names of all our students in the list below were sent to the Provincial Health Directorate and recorded in the internship software of the Provincial Health Directorate. If your name has not been forwarded to the institution yet;
You can ask the person(s) responsible for the training unit of the hospital to check the internship software or to contact the regional directors of the Provincial Health Directorate.
