 About 2022-2023 Summer Internships

Attention to Students Who Will Do Summer Internships in State Hospitals Affiliated with the Istanbul Provincial Health Directorate in the 2022-2023 Summer Term!
 
Students who are deemed appropriate to do a summer internship by the Istanbul Provincial Health Directorate in the Summer Period, FIRST (the information of these students is included in the list below, please check) must first obtain the documents required for internship registration (OHS, Single Physician Report, etc.) from the relevant hospital or the website of the relevant hospital. They must complete their applications by submitting all required documents to the hospital where they will do their internship, before the start date of the internship.
 
The insurance entries of the students who will participate in the Summer Internship, which we place in public hospitals affiliated with the Istanbul Provincial Health Directorate, will be made by the school, and they can check their insurance entry documents via E-government as of Monday, 03.07.2023. Students whose insurance access documents are not visible in the e-government system can request an insurance entry document by sending an e-mail to pakbulut@gelisim.edu.tr from the corporate student e-mail address.
 
Accordingly, for our students to complete their procedures, they can come to school with the "Compulsory Summer Internship Form" prepared in 3 copies on the computer (during working hours every weekday) and the "Internship Contract" if the hospital requests it, and ask the Program Head (from the SHMYO Internship Commission on the days when your Program Head is not at school). (Block C) they are required to obtain a wet signature. After getting the signature from the Program Head (Block C), you need to get approval from the SHMYO Directorate (you can apply to the 2nd Floor Secretariat of Block C), go to the hospital with the insurance entry document obtained from the Accounting Department and get a signature and stamp on the internship forms. After receiving a signature stamp from the hospital; One copy of the internship forms must be submitted to the student affairs office, one copy to the program head, and one copy to the hospital where the internship will take place.
 
ATTENTION! The names of all our students in the list below have been forwarded to the Provincial Health Directorate and recorded in the internship software. If your name has not been submitted to the institution yet; You can ask the person(s) responsible for the hospital's training unit to check the internship software or contact the regional managers of the Provincial Health Directorate.
 
*Students who want to cancel their internship can notify us within 3 business days by submitting a petition to shmyostaj@gelisim.edu.tr. Late cancellation requests WILL NOT BE ACCEPTED.
 
*For Istanbul; Any internship documents sent via e-mail to shmyostaj@gelisim.edu.tr will NOT be accepted.
 
*If you have problems with the execution of your internship, you can get information from your Program Head or shmyostaj@gelisim.edu.tr.

2-Internship Contract (Must be prepared upon request of the hospital. State Hospitals require it.)
